Paper Survey Fees: Many organizations prefer to use paper surveys instead of the online option. Choosing the paper survey option will require you to pay an additional fee per the chart above.

Employee Feedback Data Dashboard: (optional)
After we have collected the responses from your employees, that data is available for purchase in the Employee Feedback Data Dashboard. Answers will be provided to all questions asked on the employee survey in the form of percentage of positive response. Open ended comments and benchmarking data will also be available. The Data Dashboard is an interactive tool where data can be viewed in charts, graphs and downloadable pre-created report formats. Ordering your report package by May 10, 2024 saves you 15% off the price!

Withdrawal Fee: Any organization that wishes to cancel participation or withdraw from the program must do so by May 10, 2024 (registration deadline). Any withdrawals or cancelations received after that date will be assessed a $199 withdraw fee.
